Thank you for your interest in the Emergency Assistance for Livestock, Honey Bees, and Farm-raised Fish (ELAP) program. Before you make an appointment with your local USDA service center, use this tool to help you gather the information and documentation needed to apply for the program.

Your responses and uploaded documentation will be emailed to you so that you can easily forward them to your local USDA service center. You can look up your local USDA service center and their contact information here.
A Farm Service Agency representative will then be able to review your submission and let you know if any additional documentation or information will be required in preparation for your in-person meeting to submit your formal application to ELAP.
